Abstract

Background: Surgical pulmonary endarterectomy (PEA) is treatment of choice for operable chronic thrombembolic pulmonary hypertension (CTEPH). Atrial fibrillation (AF) or atrial tachycardia (AT) are frequently observed in early postoperative period.
Objective: The aim of the study is to identify clinical significance of early postoperative AF / AT (less than 30 days).
Methods: A retrospective study of 197 patients (mean age 60 ± 12 years; 62% males), who were treated for CTEPH with PEA at a single centre between 2003 and 2017, was performed. Median (IQR) follow-up was 4.2 (1.3; 6.4) years. Patients with early post PEA AF / AT were compared to patients with CTEPH without diagnosis of any AT / AF and those with AF / AT not related to PEA.
Results: The prevalence of AF / AT in early post PEA period was 10% (20 patients). The prevalence of AF / AT without relation to surgery was 29% (57 patients). No statistically significant difference was identified between patients with manifested arrhythmia only in early post PEA period and without any arrhythmia, more details are in Table. Patients with early post PEA manifestation of AF / AT had lower LA size (37 (34; 12) vs. 45 (40; 50) mm; p = 0.0002) and more frequent NYHA I class (10 (50%) vs. 15 (26%); p = 0.048) than rest of patients with AF / AT not related to PEA. The difference in postoperative PASP between patients with early post PEA arrhythmia and rest of AF / AT patients stayed just without statistical significance 38 (35; 46) vs. 45 (38; 71); p = 0.06.
Conclusion: The occurrence of AF / AT in early post PEA period seems not to be clinically significant.
