Abstract

As patient populations advance in age, recommendations have been made about which patients should be targeted for conversations about their personal wishes in regard to end-of-life care. However, according to the CDC, 70% of Americans have had no such discussions with their healthcare providers, nor have they named who would make medical decisions for them should they become unable to do so.
In this session, residents from Unity Healthcare, a Federally Qualified Health Center, in Washington, DC, discuss their efforts to optimize Advance Care Planning utilization in the outpatient setting. The presentation will include a brief background of the need for the intervention, an overview of the education healthcare providers received, and the effective implementation of these skills as reflected in EMR documentation.
